Hi,

I have a PDF copy of the employee rate authorization form. Does anyone know do we need a new original authorization form for each hotel we stay in? I noticed there is some numbers on the bottom left corner of the form now, but the old form doesn't have that.

Also, since I'm receiving the explore rate discount from the associate, can the associate track or have history of where I stayed each time?
