I personally haven't used Carmel in several years. I live off the A train in Brooklyn so if traveling on my own dime the extra time (about 15 extra minutes when compared to a car service) isn't worth it to me compared to the $7.50 AirTrain to Subway option.
When traveling for work I either use Uber or a GroundLink reserved ride.
Dial7, Carmel, All State, Blacklane, GroundLink and a few dozen others are a myriad of options. You'll have to check to see the rates each offers for your needs and the general reviews. Just avoid SuperShuttle or a shared van option as they will load extra people in and you can be the last stop.
