1. difficult to tell - I haven't received such an offer before but it's been a while since I joined. It might be that currently everyone new gets it. They have run a few 'triple avios' offers recently which ran for 3 months at a time so I'd say there's good chance of those sort of offers coming around again.

2. in the triple avios offers, it's been the base mileage before cabin or tier bonuses.
