If it's a choice between those three, I'd go with MK, which is a well known company with good quality cars and drivers.
That "fixed fare" table on MK site is misleading, as it does not include expressway tolls. In addition, they apparently charge an automatic 2000yen extra for pick-up at HND. Assuming they take Wangan route, it'll probably end up being almost 5000yen higher than the listed "fixed fare" when all is said and done.

Imo taking the Limo Bus is a no-brainer unless the cab is on someone else's tab. If it were up to me, I'd take the Limo bus even if somebody else is paying, as I find that a taxi ride starts to get suffocating after ~30min.
