OK, if you are going to include helicopters, I may well be the only Flyertalker to fly BFI to Victoria Harbour (Camel Point) Heliport CBF7 (no IATA code).
Helijet do call themselves Helicopter Airline, and do fly regularly scheduled flights, so guess that makes then an airline @:-)
I suspect a few of us have flown to YWH (Victoria Inner Harbour).
Even shorter lived BFI to YWH, don't recall airline, but probably Harbour Air on one of their (back then, don't know if they still land on land or just water) planes with wheels.
Not all that exotic, but short lived routes where FTers may not have flown
