From what I have read, this is the first time a passenger has refused an IDB situation & been removed with force on a UA flight. From that perspective I call this an isolated incident. If it is the first time that this exact situation has happened, then it is not a commonplace incident.
All of the other customer relations issues/incidents that people ancedtodally refer to are not this incident. I get that some hate UA, and looking at many commentators history, it is obvious that some (not all) have limited history with the airline and may well have never flown on UA. I am not saying they can't comment on these posts, but I definitely give them less weight if they are just repeating what they read/heard somewhere on the internet and have no experience dealing with the airline and their employees.
