I purchased two I/O fares in the last month and both had the link to request upgrades. I've never seen a "form" automatically appear. Rather, I've only seen a link on the purchase confirmation page that says "Request Complimentary MVP/G Upgrade" which takes you to a form. Are you saying that there was no such link?
