Thanks for your answers. My stays have only been in Europe, and a couple of times at Doubletrees I have been asked for the name of the second person in the room (there is a space for it on the check-in form).

In terms of breakfast vouchers, only once have I been given these, instead the breakfast staff know Gold/Diamond status from their printout. I don't remember being offered a smaller breakfast than other guests.
