I don't see the words "fee" or "add/collect" anywhere. It's listed under "Additional Charges" and is described as "Premium Cabin Upgrade." It presents this way on two reservations I currently have - 1) a ps EWR-LAX flight and 2) a PHX-EWR flight. The first one ended up booked into United Business (P) class and the second in United First (P) class.
