One problem with C+ being an "upgrade" is that the system auto assigns all seats, including middles, when upgrading. This means a middle bulkhead is >90% chance of being occupied. That doesn't stop you or your companion offering to switch the middle person to a window or aisle seat instead. It is totally fine to do this during boarding.

Bulkhead seats are very good on the MD90, B737 and A319 as there is no actual bulkhead wall and lots of legroom. The B757 ... depends on the configuration.
