In addition to the two items mentioned by terenz, when airlines merge the accounts get merged. When Pan Am went belly up the miles transferred to Delta. Recently folks had an opportunity to get free 1500 AA miles, by signing up with Reno who offered a 1500 mile signup bonus. The miles later merged to AA. And there is some speculation about USAirways miles transferring to United if the two airlines merge.
