I just booked a friend on a MFR-CLE flight using miles and reversed his first and middle names (he typically goes by his middle name). Now this was obviously all domestic travel. Didn't notice the switch until arriving at MFR. He said UA didn't care but he had trouble with the TSA. Secondary search, additional questioning, extra check of ID information and travel docs, etc.... He arrived early so he made his flight.
