T-REX in Denver should be included in this list.

I agree with CluebyFour about the Schuylkill Expressway - not because the road's that bad, but it's home to some of the worst drivers in America.

Finally, I-80 at the Chicago / Indiana border is terrible at rush hour, and pretty ugly at all other times.
