Echoing others here it does help if you get the mid-J cabin larger bathroom. Plenty of room in there. Step on tops of shoes etc when changing and what not.
Depending on how quick and deft at the maneuver you are you could also swap out pants under the blanket at the seat. I've done that 2x when one lav wasn't working and it was a mad rush before landing to the others. Of course I'll say that gym class for grades 7-12 at an all boys Catholic school has honed my gym room changing technique.
I would still never change my shirt if it meant being bare chested.
