They are often self-declared. Assuming you're connecting directly to a domestic flight to CNX:
1) Head to the domestic transfer immigration area upon arrival in BKK which is at the A/B/C/D concourse confluence.
2) Once disembarking the plane in Chiang Mai follow signs to 'international baggage'.
2a) If you have no checked luggage simply keep your mouth shut and walk toward the 'domestic baggage' area and exit thereby skipping customs.
3) Upon getting your bags you'll exit through customs which is largely a big nothing.
If you're itin is on PG they'll give you a sticker to wear indicating you are connecting so the agents at the domestic airprot can pick you out. However there's no restriction for only those people to access the international baggage area and I don't think *A itins typically offer those types of indications.
