An extra seat for persons of size is accomodated via a surcharge with the passenger flagged with a special needs code. In such cases the passenger does not have two bookings.

Having two bookings for the same passenger creates all sorts of issues for manifest reporting to immigration authorities, load calculation etc so is typically disallowed by any airline.
