I put accessible in quotes because the ACAA definition of accessible is only that one be able to maneuver the tiny onboard aisle chair into and out of the lavatory, not a large "fully equipped" restroom that would be found in areas governed by the ADA.
That's a key. There are still widebody aircraft out there that have not had a full interior retrofit and were delivered before 1992. No accessible lavs on those birds. The entire Delta 747 fleet is one example.
Pretty despicable IMO, but legal.
