Same way it gets a .com or a .net or any of most other TLDs: pay cash. Most TLDs are not restricted to specific groups or industries; it is just recommended. Some (.edu, .travel, etc) have rules but even within those the level of enforcement can vary.

No and no. It is mostly about CC sign-ups. So you pay to receive emails with CC links in them.
