There is no national law regarding smoking at public places in Japan, it is up to each town, municipality, etc.
As far as Tokyo 23 wards is concerned:
Chiyoda ward, Oota ward, Itabashi ward, Chuo ward, and Shinjuku ward, ban outside smoking completely. However, for example, Shinjuku ward has an ordinance prohibiting smoking outside but there is no penalty specified on the ordinance.
Suginami ward, Shinagawa ward, and Setagaya ward have an ordinance banning outside smoking at specific area only.
Many restaurants in Japan still have smoking section.
