My guess is that both are prohibitively expensive. High attrition doesn't help the math on 'investment' in training. "Basic manners" is also a very relative concept, especially in the British mindset.
It all seems to be about cost, not thought given to how to raise revenue or retain high spend customers.
Only if people leaving actually happens, things may change. It is much like a hotel with a 97% occupancy rate. You may not like the room service, but they don't care.
