Final follow up. I was ready to consider this closed but received an email this afternoon offering the additional $398 refund. Very surprising to see that go through.
End of the day I got the $964 I requested plus 30000 skymiles.

I'm glad you pursued this. I have also received what I requested (an amount more than yours). I had to pursue it over and over, but in the end they agreed I was being reasonable.
