Professional activities is one of the 12 check boxes in accordance with US government regulations, currently usually on the same sheet of paper as the application for the Cuban tourist card.

No, you need to present a PINK travel card in order to board a flight from the US. Your airline or its rep agency sells them, varying from U$50-100 depending on who's selling them.
