Cape Town does have a suburban rail system. I'm surprised you think every large city in the developed world has a decent and safe suburban rail system except Cape Town.
What about Detroit, San Antonio, Tampa, Orlando, Cincinnati, Indianapolis to name a few just in the US which don't really have a suburban rail system? Then there are plenty of cities that I would consider the rail system as unsafe as Cape Town..
