I have never, ever seen this happen. And if you know me, you all would certainly hear about it if I did!
I suppose there's an outside chance that you have a pair of GPU passengers who didn't split their PNR and there's only 1 seat left for upgrade. But I think they would probably call the paying passengers up and offer them to split before they put a non-rev in the BF seat.
