I have had plenty of stays at many hotel where the front desk (at check in) forgot to code the key for lounge access, or it somehow deactivated. They have always been remedied by a trip to the front desk. I'd give the above 99.99997 odds of what occurred.

I haven't ever experienced my key not being programmed for the lounge when I was supposed to have access. I wasn't sure enough about perks of status with SPG to really push the issue. Lot to learn, I guess.
