Ever see a 777 or 747 take off from SAN? It has to cross the active runway twice, forcing all takeoffs and landings to be halted, on its way to the east end of the runway, because the taxiways are too close to obstructions at the edges of the airport. (Landing isn't a problem, because with the prevailing winds you end up at the west end of the runway, and there are no obstructions between there and the terminal.)
