Might be, but apparently there was some anomaly with one particular transaction and that was the reason why I asked the question.

I have following transactions posted on that card:

03/05/2016 17:35:32 Cash Disbursement $67.67
12/05/2016 01:06:24 Cash Disbursement $67.67 Released
12/05/2016 01:06:24 Cash Disbursement $70.09 Settled
That transaction was made overseas over the counter. The first row is the transaction at the moment when it was run, second two are revert and -re-posting with the final amount which incorporates FCT fee. Here is how normal transaction with overseas currency looks like:

12/10/2016 11:59:59 Cross Currency Conversion Fee - PANERA BREAD #4473 - MILLBRAE $0.80
12/10/2016 11:59:59 Purchase - PANERA BREAD #4473 - MILLBRAE - (23.29 USD @ .728) $31.99
13/10/2016 01:55:14 Cross Currency Conversion Fee - PANERA BREAD #4473 - MILLBRAE $0.80 Fee Reversed
13/10/2016 01:55:14 Purchase - PANERA BREAD #4473 - MILLBRAE - (23.29 USD @ .728) $31.99 Transaction Reversed
13/10/2016 01:55:14 Cross Currency Conversion Fee - PANERA BREAD #4473 - MILLBRAE $0.80 Settled
13/10/2016 01:55:14 Purchase - PANERA BREAD #4473 - MILLBRAE - (23.29 USD @ .726) $32.08
So the first two transactions are the transaction with the spot rate at that moment, plus separate entry for FCT fee. Both transactions will revert the next day/day after and re-posted finally. Exchange rate might (will) be different at the day the final transactions are posted and it can go either way.

As you see, contrary to 'normal' transaction, in the transaction posted earlier there is no information about the merchant or the nominal amount in the local currency and there is no the exchange rate either. There was no corresponding entry which accommodates overseas transaction - cross currency conversion fee, just this entry only.

Another data point. When I another card at the same place similar entries have appeared in the transaction list, plus I was hit with $15 cash advance fee:

06 AUG PBG CASH ADVANCE 79.67
08 AUG CASH ADVANCE FEE 15.00
That fee was reversed once I called the bank and explained the situation, but the bank told me that transaction was coded as 'cash advance'.

So - go figure...
