I think you should take that Hertz offer. Yes, it's expensive but the best way to get to Milan airport.
Keep in mind, the Autostrada (traffic, narrow, tunnels ...) to Genoa and around Milan is a little "wild".
BTW, Santa Margherita Ligure, is very nice, have fun. Due to heavy traffic the street to Portofino is often closed...
