I would write into AA on the CR form on the website with the story that you gave above, minus any extraneous comments like the ones about the attitude of the CSR. That stuff is fine for FT, but would not be helpful in getting a resolution.

If you have not already, you should double check with your credit card company to make sure the charge was not reversed. Sometimes you get a charge on one statement and the statement for the following month shows a reversal.
I don't entirely see the story saying the upgrade was both ways. BUT if that's true then I agree. You have to pay the miles and the $350 twice. If that's the case then yes you owe AA another $350
