Not quite true. I've had Xs twice when it's had nothing to do with APC answers! If your name matches a name on a list, you get an X. It's not me guv, honest!
The first time I used an APC machine, I got an X. As I had a friendly officer I asked him why and he said you spread your fingers too wide on the pad, keep them closer together next time and you will be fine. I have never had an X since.
