No, it was meant in a very derogatory way in the sense that the term shyster is typically used. My second wife was very jealous of my deceased first wife (second wife was just a nutcase on this matter), and she was always dissing the family of my first wife. (Of course, I was dealing with my first wife's family because my two children were the niece of nephew of my first wife's brothers and sisters.) The reference to Hubei people having 12 heads was clearly designed to be insulting in the context that my second wife used it.
Sounds similar to the idiom "talk out of both sides of their mouth".
Still may have some reference to the 12 zodiac animals, in that they change
themselves in order to influence what others perceive them as being or how they present themselves...depending on what's in their best interest. ie: 2 faced
