I believe if you book with Avis in the US but do so stating you live in the UK and you don't use any random CDP codes, it will include full insurance.

BA Holidays bookings for cars in the US also include full insurance, with a zero deductible.

BA Holidays bookings for cars in Europe do have a deductible - It was the best part of a grand when I rented in Norway!
