Row 45 has tons of legroom, it is by a full size door.
E+ is not like AA's MCE, but a separate cabin class. A move from Y to Y+ is an upgrade, so unless they are overbooked or you buy it at check-in, not going to happen.
JAL does keep a number of seats blocked to assign at the airport, they are typically assigned to status members without a seat reservation, or people asking for them. If you come to the airport early and ask there is a good chance. The first rows of economy class, the middle bulkhead after galley and the rear rows are normally blocked. Mind you, some of the bulkhead seats are also bassinet positions, so will only be free if they do not need them for that.
