Boeing actually planned for it to have decent seating. The airlines are the ones that instead packed extra seats in.
From what I heard the 787 team specifically planed 3-3-3 but publicly advertised 2-4-2 to not get stopped by the 777 team. In other words it was always 3-3-3 except for politics and a few airlines. (This could well be completely made up though, who knows...)
