I hadn't received my gold kit either so I called an requested it (I'll be transiting Dulles with a 4 hour layover and wanted to make sure I have no issues with access to the Lufthansa and Turkish clubs). My kit came with the card, luggage tags and literature like the previous poster said but it also came with a card for my MM designee as well.
