Given NH had the veg request, I can only imagine it was requested properly at booking, but dropped with upgrade.
I wouldn't disagree with the need to reconfirm - I find special meals drop randomly. I now always reconfirm at check in as then I know whether I need to buy something for the flight - useful at LHR where it's easy to find lots of GF options, but difficult at SFO which has very limited choices.
