I think what's going on is that US-LHR isn't a market where DL is selling C+ as a separate W fare. I did a test search and on "view seats" it has the side blurb about paying $99 for C+. So if you want C+ on those flights I think you book a regular main cabin ticket and then choose for free or pay according to Medallion status.

The reason there *are* flights with choices in the Premium Economy column on that search is because some of the itineraries are VS-operated DL codeshares.

(... because all the C+ changes aren't confusing enough, or something)
