The thread title poses a question, it doesn't a assume a unique explanation.
Coincidentally I just went through a similar process, again reflecting the airline's reluctance to part with cash:
- only online cancellation permitted
- after some weeks ....wildly inaccurate refund (in QR's favour
)
- call centre agent agrees the result is absurd and sends the request back through the system
- after some weeks .... corrected refund received.
