The thread title poses a question, it doesn't a assume a unique explanation.

Coincidentally I just went through a similar process, again reflecting the airline's reluctance to part with cash:
  • only online cancellation permitted
  • after some weeks ....wildly inaccurate refund (in QR's favour )
  • call centre agent agrees the result is absurd and sends the request back through the system
  • after some weeks .... corrected refund received.
