I had booked via the internet (paid for my Ticket in Rp), and my departure booking (eticket and PNR) had no mention of any departure tax. The first time I found out about it was at the checkin counter, when the agent asked for 150Rp per person. After a surprised grumbling from myself, I paid up and a receipt was issued. I did ask what would of happened if I had no Rp's left - the response was, that they had electronic EFTPOS available.

Truly, this departure tax is a best kept secret.
You may want to check with QR whether this was real. The premium tax is 9% and is part of the ticket. It isn't supposed to be collected separately.
