The total fare would have had to have included the $5.60/direction 9/11 fee ($11.20 total), 7.5% tax, US segment tax, and passenger facility charge. The last two on the MIA-LGA-MIA route are $8 and $9, respectively. That leaves the return fare of $162.
If the return had been calculated based on fare, the OP would've earned 810 base miles, 486 bonus, for a total of 1296 miles (instead of 1756).
The OPs credit looks very much like my recent R/T JFK-CLT. The discrepancy in my case was because the outbound was in L class and the return in N class. Doesn't explain the OP's missing 36 miles though, unless that was some overlooked tax of $7.20.
