Flights booked online are ticketed instantly.
Most people book simple round trip itineraries online.
And AC doesn't cancel unticketed segments, so not only does it need to be booked over the phone, but it needs to involve other airlines (and I don't know if ALL airlines will cancel the reservations, or just certain ones).
So while I agree they need to fix it, I don't think it's nearly as broad an issue as you suggest.
