Getting land side will depend on what passport you have and luck, it could be fairly quick - perhaps 30 minutes on a good day. Getting back to air side can be about the same. It all depends on the day and the time of day. It may take a while to check in if you have to.
This is what they have to say about T5-T4:
Take bus number 482 (destination: Southall Town Hall) or 490 (destination: Pools on the Park) from Bus Stop 7 for a free transfer between terminals.
Average journey time: 20 minutes

You can also take a free train to T1/2 and then another train to T4 or use the Tube if you have the right card. These will likely be slower.
