Most people don't, although it's been written up in travel articles as great advice to have a change of clothes with you just in case. Another piece of great advice is when you are traveling with someone else, divide each person's clothes up between the suitcases. If one goes missing, at least everyone will have clothes for a few days until the suitcase is recovered.
As far as ripping clothes anywhere, yeah, you may not be within 15 minutes of home (heck, with traffic, that's only a few miles from home!), but you're probably going to be close to some sort of store that sells clothing. When times allows, quickly run out to get something.
