Yes there may be charges for additional guests up to that maximum - and this would cover extra cleaning, breakfasts etc. - but they would be clearly indicated in the booking confirmation and agreed to.

So if I'd searched for rooms for 4 adults, booked a room or suite that said "4 person(s) per room maximum" with whatever extra charges (if any) were detailed, then I'd be expecting to receive a room that catered for 4 people, including beds, and also breakfast or whatever else was included in the rate booked, without having to pay any extra when I got to the hotel. If the hotel has made a mistake with the capacity they entered in the IHG system for that room then that's for them to resolve, not the guest(s), and I'd be invoking IHG's reservation guarantee if they tried to say otherwise!
You have described precisely my feeling on the matter.
When I make a booking for a rental van, which seats 7 adults, should I not expect that it effectively comes with 7 seats? And that I don't need to pay a fee at pick up to add any missing seats to come to the 7 of the original booking?
I really don't see how it could be up to the discretion of the hotel to come up with anything different if a room is listed as suitable for 4 adults, and is confirmed in a reservation with a quoted total price.
