On ultra long haul (say, two back-to-back 12h flights) the recline and legroom make a real difference;

Separate cabin with better service and much quieter than the main Y cabin.

If you compare it to bottom-of-the-barrel Y then yes, it's awfully expensive; if you compare it to a semi-flexible fare it's a modest price hike for tangible benefits.
