We're going on a cruise so I will have to check a bag (I know that's blasphemy here!). We travelled around Italy, Germany and Spain on a train with luggage and that made me decide against FCO/MXP + train.

The BA flight from PHX costs an extra $475 and possibly extra miles (as I indicated above, I booked this with Alaska miles on AA metal - not sure how they will handle a reschedule that doesn't follow the rules of Alaska's program but I doubt either AA or Alaska will be willing to concede too much when there are other less expensive options for them.) I plan to call Alaska again tomorrow and see if any progress has been made on "the queue for resolution", whatever that means...
