Some of our recent experiences:

Hyatt Regency Tokyo - Club King Room allows up to 3 but they do not allow a rollaway bed. They provided bed guards and our (toddler) son and he shared the bed with us.

Grand Hyatt DFW - rollaway allowed in regular King room

Hyatt Regency Dallas - hotel was sold out so we were upgraded to a presidential suite. It was nice to have the extra space but there was no good space in the bedroom to fit a rollaway. There is a sofa bed in the second living room that is on the opposite side of the suite that could sleep 2.

Andaz Papagayo - regular King room allows a rollaway. Cambi Kids Club is free for kids aged 4-12. My son was a few months shy of turning 4 when we visited but because they had low numbers the week we were there they allowed him to stay. At the time of our visit (March 2016), kids 5 and under ate free off the kids menu, and also off the buffet for breakfast.
