The few times I've seen reports of held reservations disappearing before the hold expired was when the held reservation conflicted with another reservation by that person.
I have had a reservation put on hold then a couple hrs later I saw a better schedule for me but when I tried to reserve it-it wouldnt let me do it stating"a similar reservation is on hold please call the AA number and they then changed it to the new reservation
