I've seen this when I do a hold and I'm not on the ticket. Usually if I'm getting my wife an AAdvantage ticket out of my account or similar. The reservation is still there, but you need the PNR to find it. And AA has hidden the find by PNR for logged in people as well (made it hard to get to).
