I checked the itinerary at least 5 times. I used to have a paid Expertflyer account and would always place seat alerts on my seat to make sure this sort of stuff doesn't happen but stopped once Delta pulled away useful upgrade inventory access from Expertflyer. Honestly short of checking the itinerary constantly this could happen to anyone.
